Home Care Physical Therapist (PT) / Physical Therapist Assistant (PTA) - Merrimack Valley/North Shore
Job Summary: We are seeking dedicated and compassionate healthcare professionals to join our home care team serving the Merrimack Valley and North Shore areas of Massachusetts. In these roles, you will provide rehabilitative care to patients in their homes, focusing on improving mobility, managing pain, and enhancing overall quality of life. Collaboration with patients, caregivers, and healthcare professionals is essential to ensure comprehensive and effective care delivery within our community.
Responsibilities (for PT):
Conduct thorough assessments of patients' physical abilities, medical histories, and home environments to develop personalized treatment plans.
Provide skilled physical therapy services including therapeutic exercises, manual therapy techniques, gait training, and balance training.
Educate patients and their caregivers on proper techniques for exercises and activities to promote recovery and maintain functional independence within their home environment.
Monitor and document patients' progress, modifying treatment plans as necessary to achieve therapeutic goals and ensure patient safety.
Collaborate closely with physicians, nurses, and other healthcare providers to optimize patient outcomes and ensure continuity of care.
Maintain accurate and timely documentation of patient evaluations, treatment sessions, and progress notes in accordance with regulatory and organizational standards.
Adhere to ethical and professional standards of practice, respecting patient confidentiality and safety protocols specific to the Merrimack Valley and North Shore communities.
Responsibilities (for PTA):

Implement prescribed physical therapy treatment plans, including therapeutic exercises, manual therapy techniques, gait training, and balance exercises.
Educate patients and their caregivers on exercises and activities prescribed by the supervising Physical Therapist to promote recovery and maintain functional independence within their home environment.
Assist in monitoring and documenting patients' progress, reporting observations to the supervising Physical Therapist and modifying treatment plans as necessary.
Collaborate closely with the supervising Physical Therapist, physicians, nurses, and other healthcare providers to optimize patient outcomes and ensure continuity of care.
Maintain accurate and timely documentation of patient evaluations, treatment sessions, and progress notes in accordance with regulatory and organizational standards.
Adhere to ethical and professional standards of practice, respecting patient confidentiality and safety protocols specific to the Merrimack Valley and North Shore communities.
Requirements (for PT and PTA):
Degree in Physical Therapy (for PT) or Associate's degree from an accredited Physical Therapist Assistant program (for PTA).
Current state licensure or eligibility for licensure as a Physical Therapist or Physical Therapist Assistant in Massachusetts.
CPR certification.
Must pass a background check.
Clinical experience in a healthcare setting, with preference given to those with experience in home care or community-based settings.
Strong interpersonal skills, with the ability to communicate effectively with patients, caregivers, and healthcare team members.
Ability to work independently and manage time effectively in a home care environment.
Compassionate and patient-centered approach to care, demonstrating sensitivity to cultural diversity and the unique needs of the Merrimack Valley and North Shore residents.
Preferred Qualifications (for both PT and PTA):
Knowledge of Medicare and Medicaid regulations related to home health services in Massachusetts.
Working Conditions:
Provides care in patients' homes within the Merrimack Valley and North Shore areas, with travel between appointments required.
Flexible scheduling allows you to choose your own hours, typically Monday to Friday, to accommodate patients' needs.
Must have reliable transportation and be able to navigate local communities efficiently.
Compensation:
PT per diem rate:
Evaluations: $65 per evaluation
Visits: $55 per visit
PTA per diem rate:
Visits: $45 per visit
