Critical Care Pharmacist IIIClick Here to Apply OnlineJob DescriptionLocation: Wadley Regional Medical CenterPosted Date: 10/30/2023The Critical Care Pharmacist works primarily in a decentralized model providing comprehensive clinical pharmacy services including thoroughly reviewing patient charts to identify and correct medication related problems, pharmacokinetics consultations, drug dosing consultations, and adult parenteral nutrition management. They will be the Pharmacy Residency Program Coordinator, primary preceptor for a six-week critical care rotation for each PGY1 resident and may also be required to precept longitudinal residency rotations such as research or teaching. Availability to precept fourth year pharmacy students is also required. Will be expected to meet the pharmaceutical care needs of neonatal, pediatric, adolescent, adult, geriatric, psychiatric, oncology, obstetric, chemically dependent, rehabilitation and skilled-nursing patients.EDUCATION AND EXPERIENCE REQUIREMENTS:Must be a graduate of an ACPE accredited School of Pharmacy with at least a BS Pharmacy degree.3+ years hospital critical care experience, or PGY1 residency; PGY2 residency preferred.Must be currently licensed, or eligible to practice pharmacy in the state of Texas.Familiarity with hospital clinical committees.Able to perform formulary reviews and medication utilization evaluations.Familiarity with pharmacy residency practices preferred, including ASHP accreditation standards and the functionality of a Residency Advisory Committee.Equal Opportunity Employer Minorities/Women/Veterans/DisabledApplication InstructionsPlease click on the link below to apply for this position.
